Modified EHRA Classification of AF-Related Symptoms

Standardized grading of AF symptom burden. Guides the decision between rate control (EHRA I–IIa) and rhythm control strategies (EHRA IIb–IV). Based on the modified 2017 EHRA classification.

I None
AF does not cause any symptoms. Normal daily activity is not affected.
IIa Mild
Mild symptoms related to AF. Normal daily activity is not affected by symptoms.
IIb Moderate
Moderate symptoms related to AF. Normal daily activity is not affected but the patient is troubled by symptoms.
III Severe
Severe symptoms related to AF. Normal daily activity is affected.
IV Disabling
Disabling symptoms. Normal daily activity is discontinued.

Common AF Symptoms

Palpitations, fatigue, dizziness, dyspnea, chest pain/tightness, anxiety, exercise intolerance, and sleep disturbance.
