Prior major bleeding or predisposition to bleeding

Labile INR
Unstable/high INRs, time in therapeutic range <60%

Elderly
Age >65

Medication usage predisposing to bleeding
Aspirin, clopidogrel, NSAIDs

Alcohol usage
≥8 drinks/week
