INTERMACS Profile

Advanced Heart Failure Severity Classification (Profiles 1–7)

Select the profile that best describes the patient's current hemodynamic and clinical status. Used to guide timing of LVAD implantation and transplant listing.

Clinical Notes

  • Modifiers: TCS (temporary circulatory support), A (arrhythmia), FF (frequent flyer — recurrent HF hospitalizations).
  • Optimal LVAD timing: Profiles 2–4 have the best outcomes with durable MCS. Profile 1 carries high surgical risk.
  • Profiles 5–7: Generally too well for LVAD — optimize GDMT and reassess over time.
